GIS Request Management - Business Unit select_one not working

In the GIS Service Request Survey123 XLSForm I have defined the BusinessUnit choices as:

On the Survey tab this is defined as:

In the Survey123 Connect app, this displays correctly:

And yet on the ArcGIS Hub - GIS Service Portal page where I can submit a request - the form there doesn't work. The only option I have is 'Other':

How can this be corrected?

Did you publish the updated form in Survey123 Connect?

@OliverIshmael - Like Josh mentioned, check to see if you've published the survey in Survey123 connect. Also, double check that the syntax of the list_name on the choices tab matches the select_one {list_name} on the survey tab.
