What do you have to do to get the Start and End Tiles to show something other than a 404 error? The tiles exist, but REST will not see them.
How do I figure out what file /tile/0/130/104 is referring to? And also what user is trying to connect to it?
I don't see any differences between this cached set and others that are working fine in the same cached directory.

Solution:
Select the cache directory in IIS.
Double-Click Authentication, Right-click the Anonymous authentication line, Edit...
Press Set... Enter ArcGISSOC and password.
Press OK.
