Is there any way to keep data private in ArcGIS online when sharing it publicly? Could I publish services on my own AWS?

Although I would like the web app to be accessed publicly, due to data sensitivity services can not be found and consumed by outside users.

Is ArcGIS Enterprise the only solution?

Pl. check using feature service views. You can keep your source feature service private and then expose a view of the source feature service with the desired capabilities (read-only for instance). Views will see any changes in the source feature service as it happens.
