I created a Field Maps layer group using the default template, consisting of Points, Lines, and Areas layers. I added this layer group to a web map that also includes other non-Field Maps layers.
I have configured the categories, domains, and code values to align with the layers' styling under Styles > Unique Symbols. The default "Category 1," "Category 2," etc., are now properly labeled to match the domain descriptions. Each feature (points, lines, areas) has specific symbols, colors, and text labels assigned.

However, the Field Notes (Lines) layer is not behaving as expected:
Troubleshooting attempted: I tried switching the styling from Unique Symbols to Single Symbol (and back), then reselecting the "Category" attribute and reconfiguring each category's line symbol and color. However, when accessing Configure > Templates in the web map, there is no prompt to sync changes, and the lines remain gray.
I've attached screenshots for reference:
TL;DR: The unique symbol styles (colors) for the Field Notes (Lines) layer are visible in the layer properties and legend but are not propagating to the web map's Edit menu, Configure > Templates section, or the Field Maps iOS app. This worked correctly until recently, with colored lines appearing in all these areas.

After poking around in the Field Maps Designer, deleting every existing custom "Category" it finally prompted to resync at which point it repulled the symbol and color renaming them to default "Category 1", etc. after renaming and checking the main Main Web/edit/etc. the colors had returned.

This sounds like a classic Feature Template sync issue. Even though your legend looks right, the "Edit" menu and Field Maps app pull from the layer's saved Templates, which can get "stuck" on old or default styling (those gray lines you're seeing).
Since you aren't getting the auto-sync prompt, try this:
Manually Reset the Templates: Open the Map Viewer, go to the Edit pane on the right, and click Manage Templates. Select your Lines layer, delete the current templates, and hit Add New. This usually forces the map to "scrape" your current Unique Symbol colors and apply them to the editing tools.
Check the Field Maps Web Designer: Go to the Field Maps web app (not the mobile one) and open your map. Look at the Content tab and click on the Lines layer. If they look gray there, you can fix them in the Templates section of that designer. It's often more reliable for pushing changes to iOS than the standard Map Viewer.
Layer Groups Bug: Since this is a Group Layer, try dragging the Lines layer out of the group, saving the map, and checking the Edit menu. If the color returns, drag it back in. Group layers can sometimes "mask" style updates from reaching the templates.
Give the "Manage Templates" reset a shot first—that usually clears the pipes.
