Survey123 Widget in EB not connecting to service

Hello, I created a survey from an existing feature service using Survey123 Connect. I would like to send data to the survey however, I am not given the option because the Add Connection button is greyed out. I am given the option to Add Connections if I connect to a different map widget and layers within the app. Am I missing a setting or action somewhere else within the app?

After submitting this post, I quickly realized it was because my layers were grouped. After I ungrouped them in my web map and refreshed experience builder, everything worked as expected.
