Multi Pick lists in Collector

Is there a limit to the number of attributes that can be collected/displayed in Collector? I made my domain with 15 attributes to collect, but when I get to the app there are only 6 displayed. How do I get to the rest?
Also - I added coded values in my domain, so that they user can only pick what I coded (for data integrity and searching database later on), how come I don't see this?

You can create a group and share the service to that group and invite Collector4ArcGIS into that group and I can check it out without having to make the service public.

Are you using iOS or Android? I did a quick test on iOS and I am seeing all the fields and domains listed.
