Did you look at this sample ? It's managing print to scale (may be multi pages). Paper size is just a matter of styling. With the default styles, the output strechs to fill the print area size (so depending on the printer driver, the orientation, the paper size, ...), but nothing prevent to create a style with a fixed size.
