Attached is an XLSForm that seems to validate OK. I can upload it into ArcGIS Online, and it creates the survey OK. However when I try to fill it in, and submit, the submit button does not work. Even though all the required fields are filled in....
I've checked the feature service to ensure that attachments are enabled and that editing is also enabled.

It looks like you are using the an incorrect value for instance_name.
Please review this link for more info and I strongly recommend read this blog to take advantage of this feature.
