If you haven't seen this already, here's an ESRI blog post which provides guidance about hosting your own story map. I haven't done this yet myself, though.

I don't think this is possible, but there might be a way to combine your existing application with the one downloaded from Github by using the ArcGIS Online Assistant to view the app's JSON.
