Survey123 Form- Error initializing form- and hyperlink list

Question

I am working with ArcGIS Enterprise Portal 11.1 (Development) , and survey123  version 3.18

My goal is in my survey form is to show a hyperlinked images’ list that are stored in my enterprise account.

Here are some screen shots for better understanding of my problem.

Note. everything works beautiful on Survey123 connect, the problem is when the form is published everything gets wonky.

Hi

I try to avoid using html coding in choice labels.

I would suggest rather using different notes that you show/hide depending on the selected option?

Hopefully that doesnt clutter your UI too much
