Survey123: Failed to Save - Undefined

Hi,

I am getting an error on a survey that has been around for a while and is working properly.
I have 20 fields that are all defined the same (!), a field that can be entered as a value between 0-5 and I got an error on most of these fields this week and now I can't make changes to the survey and save and publish it, I get an error on all of these fields with the field name and "undefined".

-All fields are marked as required to answer
I would appreciate help because I am stuck with upgrading the survey.
