Date field in Published survey has changed

Hi,

Anyone come across this before?
We had a date field in our published survey that has now changed in appearance?

The survey was designed using the web designer.

We republished the survey and it seems to have come right now. The weird thing is the survey was never edited so why the appearance changed is still a mystery.
We even checked it on multiple browsers and we had the same issue.

I wonder if there was something updated in ArcGIS Online that would have caused it.

was this survey created in Survey123 Connect or the web designer? Can you share all the settings for that question?
