I know there are many other posts about this error, going back to 2021. I've read most of them. Haven't found a solution that seems to apply. Hope someone out there can help.

I'm working with Survey123 Connect Version 3.21.62. Created lovely survey and it looked fine in the Survey 123 Connect preview. Saved & published the survey. Selected "create web form." Get dialogue box that says "publishing complete". From Survey 123 Connect, ran the "convert choice lists to CSV" and "Update XLs Form template". The saved folder for this survey shows a webform file. Strangely, our organization's Trust Center gives the Application add-in disabled" warning, though I'm not sure that's relevant.

Did you move the files to a new folder in the Portal after publishing from Connect? I'd think this is the most likely cause.

When publishing, did you un-check "Create web form"? (This is checked by default. But if you manually un-check it, I actually don't know off-hand if it re-checks itself for subsequent new publish attempts)

Hi @abureaux ! Thanks for the reply. This problem was actually happening before I moved folders in AGO ...  but another post had me take all the files in the solution and take them out of an actual folder. I did find some solutions here that have me looking into the itemInfo properties of the folders in AGO and on my machine. So far I haven't quite broken that code though. So now I've "fixed" things into a real mess.  🙄

Also, you have more options in your "publish options" dialog box than I do:

I would advise against moving anything created by either S123 Connect or the S123 Web App. As soon as that folder structure is broken, it can really mess things up. If you can, I would suggest trying to move all the files back to where they came from. Another option would be to create a copy of this survey in Connect, and publish it again under a new name.

My bad on those extra options. Basically, when you first publish a survey from Connect, you get more options. If you are republishing a survey, you just get the single option you are seeing (which is web form, and the only one we care about in this case).
