Visual Studio 2008 Application Extension (Desktop) is missing.

I'm checking out "Getting started with the item templates" in the "ArcGIS Resource Center".

They have me starting a new project in Visual Studio and selecting down the tree as follows:  Visual Basic; ArcGIS; Extending ArcObjects then grabbing "Application Extension (Desktop)" to open the project template.

There is no "Application Extension (Desktop)" just the following (image).

Any idea how to get that template?

I'm well versed in VBA, but only a novice at vb.net.

Once you have created your project, you can add these new items by selecting Project from the Visual Studios menu and clicking on "Add New Item".
