I created tile package to add to my S123 media folder but it does not seem to be working in my survey. I when I go to the base map menu and choose the tile package all I get is a grey background.

It looks like you are using an OSM Basemap. How did you create the tile package?
Sorry about the delay i used an app called universal maptiler.
No worries. Is it GitHub - calvinchengx/maptiler: github mirror of Map Tile Cutter - Tile Overlay Generator for Google... ? What is the output format? Survey123 expects an ArcGIS Tile Package (tpk) file. These can be created via ArcGIS Pro or the Tile Package Kreator tool. (Note that the links to download Tile Package Kreator are being updated)
