Why can't I use an activated extenstion?

I received an authorization code for a trial of the ArcGIS Data Reviewer extension. It has the following status on my.esri.com:

   Total Authorization:1

   Available:0

   Activated:1

   Licensed, not active:0 

 

but the extension does not appear in my ArcGIS Administrator and when I try to activated the extension through ArcGIS Desktop it is listed as not available.
