Is there a reference guide that identifes all the differences in how Survey123 Forms must be created to successfully be published on Portal versus on ArcGIS Online?

I'd suggest moving this question to the Survey123 forum. The Survey123 team is very active there but might not see it under Implementing ArcGIS.
Am not aware of a form-focused list but the following page discusses some limitations to be aware of:
We'd need to design the forms around these limitations. I'd probably start with the version of Portal for ArcGIS then go from there.
