Connecting Survey to County Map (Influenza)

Good morning everyone,

 

I'm wondering if there is a way to accomplish this task:

 

At the state health department, we collect data from hospitals and school districts about the number of patients/kids that come in with flu-like symptoms.  We collect this information using a survey tool and then we generate reports. 

 

I am wondering if there is a way to create a survey using Survey123 that will collect the hospital/school's county FIPS number (i.e., 48151) and connect their responses to a state map (which has the same county FIPS).  I don't want to put points on the map, rather I want the map to identify the proper county and give that county a specific color based on the response (choropleth).

 

Ideally, this may would update every so often automatically as responses came in.

 

Side note: I've already created the map I want to use with the associated counties in question.

 

Is there any, at least somewhat easy way, to do this?
