If you have an existing feature service that has domains - you can use Connect to create a survey from the service. As part of the process, connect will create questions from the field names and choice lists from the domains. You can then modify the survey to meet your needs (e.g. delete and reorder questions, apply appearances etc). For more information on this process, please see:
