Customized feed back form

I have a feedback form which collects the information entered by user. I have requirements to include it in one of th story maps as a custom component. I am using Tabbed Story Map Series template.

Depending on exactly how you want to do it, a couple ideas:

- download and customize the story map app itself: GitHub - Esri/storymap-series: The Story Map Series lets you present a series of maps via tabs, numb... 

- present the story map in an iFrame and deal with the feedback form completely separately

Hi Aiad -- You can add your feedback form directly to one of the tabs of your Map Series -- when adding content choose the "Web Page" option and paste in the link to your survey form.
