How do I add a voice recording to a survey form? ( ie to record data about a location)

Hi Nevil,

Currently Survey123 doesn't have the ability to have a recording as part of the survey form.  I've logged your request to the internal enhancement log.  You might want place an idea in ArcGIS Ideas - this will help to know the relative importance of this enhancement.
