Error the username has already been taken

We are in process of setting up ADFS accounts and for a single user we are getting the attached error message.  If anyone has any ideas or has encountered this I would appreciate the help.  I went as far as removing his original non ADFS account and still no luck.

We've just encountered the same error with one of our users.

Hi Van, I sent an email to notifications@arcgis.com and it ended up being an old username that was in an orphaned state.  They said they removed the account after which the error went away on my end.  Good luck hope this points you in the right direction.
