We are currently at 10.2.2. Are there any issues with skipping 10.3 and upgrading to 10.3.1?

What exactly are you upgrading? (ArcGIS Desktop, ArcGIS Server, ArcSDE, everything, etc)
Mostly Server and Desktop.

Does the upgrade of the License Manager (LM) require an uninstall of LM v10.2.x and then a subsequent install of LM v10.3.1? Or can LM v10.3.1 be installed on top of LM v10.2.x?
No, you can install it on top of LM v10.2.x
Is that best practice? Or is it riskier than uninstalling the older LM version?

The installer will automatically uninstall your ArcGIS 10.1 through 10.3 License Manager and install ArcGIS 10.3.1 License Manager.
So in the back-end, it is doing the un-install of old LM and install of new LM automatically.
The only issue we found with changing from 10.2 (desktop, not sure about issues with server) (to 10.3 ) was that MXD's and layer files created in 10.3 cannot be opened in 10.2 - so you need to move everyone across at the same time. The workaround (save a copy in older format) is an almighty kludge.
This has always been an issue between major releases, especially for a large organization where you have many ArcGIS Desktop users. I would suggest keeping the 10.2 lyr files in place until all of your computers get upgraded if it is not feasible to upgrade all workstations (devices) at the same time.
The one issue I ran into when updating the License Manager to 10.3.x is that it is no longer supported on older Windows Servers. I had our 10.2.x LM on a Windows Server 2005 (?) I believe, and had to move it to a Server 2008 machine.

I was able to install ArcGIS Desktop v10.3.1 overtop v10.2.2 as it was a basic vanilla installation with no custom or 3rd party tools. Are there any liabilities with installing ArcGIS Desktop v10.3.1 overtop v10.2.2 as opposed to uninstalling ArcGIS Desktop v10.2.2 first?
