On a virtual server (created via Hyper-V) running Windows Server 2003 R2 Standard x64 SP2, I first installed ArcGIS Server 10.0 Enterprise Standard, uninstalled it, then installed ArcGIS Server 10.1 Enterprise Standard SP1, but failed to install Web Adapter 10.1 SP1 and got the error message "A virtual directory with the name arcgis already exists on the website, Default Web Site." (see the attached png file).

Cause: ArcGIS Server 10.0 was installed and thereafter uninstalled on the virtual server. After uninstalling ArcGIS Server 10.0, there were two left-over components in the MetaBase.xml file under C:\WINDOWS\system32\inetsrv folder:
<IIsConfigObject    Location ="/LM/W3SVC/1/ROOT/ArcGIS"> ? </IIsConfigObject>
<IIsConfigObject    Location ="/LM/W3SVC/1/ROOT/ArcGIS/Mobile"> </IIsConfigObject>

Solution: first to remove those two components, then install Web Adapter 10.1 sp1.

Solution was inspired by the discussion at http://forums.esri.com/Thread.asp?c=158&f=2394&t=266831 .
