Thank you for checking that out, Marisa. That is exactly what I was seeing, that with the choice filter in place the dots showed that the filter was working, but the choices were not visible. I have changed gears a bit by removing that functionality and limiting the survey to a single state. I guess I should mark this resolved for now, but eventually I would like this to work. I recreated my feature service under a new name, saved my surveys to new names, created a new webmap, and republished it all, and now it is submitting ok. Have no idea what was wrong, but guess something along the way got corrupted.  ... View more

Shobana, Thank you, you might have put your finger on the problem. I do not have admin access to my machine. I had our admin reinstall Survey123, and we confirmed it made that registry entry. However, when logged in as me it is not invoking that code. We think the issue is a security one, where trying to have an outside website (survey123) trigger an exe on my local machine, so I am not sure if I can get the admins to let this work.  For now, I will focus on making it work on the web, which I am still having issue with if you see my other thread.
