AutoCAD Client for ArcGIS Image Server

I am trying to use the AutoCAD Client for ArcGIS Image Server (ArcGIS Server Image Extension) with AutoCAD Civil 3d Verison 2010.  I can get it to work with an Image Service in the root folder of ArcGIS Server (http://gis.hamiltoncounty.in.gov/ArcGIS/rest/services/Hamilton_2009_test/ImageServer) by using http://<server name> but not with Image Services in a sub-folder (http://gis.hamiltoncounty.in.gov/ArcGIS/rest/services/ImageServices/Hamilton_2009/ImageServer).  Is this possible?

I have 19 Image Services and I don't want to move them into the root folder with 40 or so Map Services.

I have the same problem, when publish in subfolder, image services are not showing in Image Server AutoCAD Client. Write if there are a workaround.
