Identify Widget for Flex Viewer - No Loading of Results in Application Preview

I have added the Identify Widget found at the following address;

I have loaded two features into this tool in Widget settings and I chose the same URL for both of the feature layers which are found on my ArcGIS Server site. These are the same URLs I provided when adding these features as operational layers when beginning the Application Builder. When I preview my application the Identify Widget appears however when I click on one of the features which I added to the widget no results are loaded.

Any ideas as to what might be the issue?

   This time I duplicated your data down to symbology and the database name of GIS and the fact the DBO is the schema owner and not SDE. I also used the same featureLayer name and published it. The only Diffference is I don't have ArcGIS Server 10.1 SP1, I am one 10.2.1 here. Anyway once again I added the map service as a feature type layer in snapshot mode (like you), and it works great in App Builder 3.6 in the Preview tab. I am not sure I can diagnose any further as it seems to be a map service configuration issue on your end. Have you tried to setup the search widget and do a query on this layer?

I am having a similar problem. I was using this widget successfully with ArcGIS Server 10.0 Flexviewer 3.6 Identify Widget 3.6.2
I upgraded to ArcGis Server 10.2.0 and Identify widget 3.6.5. I can only identify if I keep Only These Layers off,  loosing the functionality of controlling what I wish to identify, what fields to display and the ability to open linked documents. I am using a replicated geodatabase, original data stored in sde 32 bit database (reason why I can't use sde directly)

   Are you using App Builder? Can you ignore anything that you have already configured and (for testing) start a new IdentifyWidget.xml?

1. Which version of the widget are you using?
2. Are you using App Builder?

This threads last post is over a year old and there has been several updates since then.
