Survey123 Summary Report - Missing Attributes

Hello,

I've developed a summary report for a Survey123 app which generates a table of values including repeats. I've noticed that one of my attributes is left blank for random rows, even though the attribute exists in the data. Has anyone else experienced this issue, and if so do you have a solution?

See screenshot below for an example - the "Area Name" attributes exist for all records in the database, however in the summary report it only displays attributes for random records...

I'm having the same problem - data filled out on mobile device, but is blank within record.   Only happening on 5 of 50 fields.   

ESRI tech support not helping, going on a month of trying to figure this out.
