Does anyone know if the S123 Report functionality can be included as an attachment with the same feature service? I am thinking there has to be something with the ArcGIS Python API to make this happen but not entirely sure.

Yes, you could use the ArcGIS API for Python to generate a report, download it to a computer and then add it as an attachment to a feature.
