Survey123 Report checked boxes formatting?

I have created a Survey123 Report template but when I go to print the report the formatting is not consistent. Some of my checked boxes are returning blue and some are not. I don't have a preference either way I would just like it to look the same across the board. 

I've based my formatting on the quick reference (snip attached), I've also attached my template and another snip of the printed report. 

 I can't figure out what is different about that one check mark not show up as blue like the others. 

Any suggestions are appreciated!

Hi @KylaCupp_SBY,

You may have already found a solution to this, but it looks like the "Are you under 25..." question is in Helvetica while the others are in Calibri. If you make sure all the questions in the table have the same font and formatting do all the checkboxes appear the same?
