Survey123 Connect: PNG image shown on Connect App but not on Actual Survey

Hello All,

It could be as simple as my type format being off. I add an image to type; note. On the Survey123 Connect for ArcGIS Application, the image is shown.

Although, when I upload the survey and try to take it online, the image doesn't show up.

My code is listed below. Seems pretty plug and play but a lot of the online tutorials are shadowed around people taking pictures while taking the survey. Anything helps, thanks!

Hi Zach - The web form will not like the spaces in the filename.

Rename the file and re-publish.
