SURVEY 123 CONNCET FAILED TO PUBLISHED

Hi, Am lawal and am new user 

Pls I try to a survey on Arcgis portal and is showing failed to create folder initially I have published the survey on Argis online and it worked fined but on getting to my portal is showing me error

Looks like the folder Survey-TRANSFORMER_INSPECTION_FORM already exists in your portal?

Also, be aware you're sharing a bunch of personal info including portal url, username etc) with that file. You may want to repost with only the relevant info from your log.
