"Missing .webform file" and "Survey is not accessible or does not exist."

I'm encountering a persistent error when trying to access my published survey in a web browser (screenshots attached). After making some edits to my XLS form via Survey 123 Connect, I noticed that my data records were no longer associated with the webform and saw the errors below. Notably, I edited the form to link to an external .csv file, and I think this might have been when the webform stopped working.

When accessing via the Survey123 site, I get:

"The survey is not accessible or does not exist. Please ensure the signed-in user can access this survey and that the survey link is correct."

When accessing from ArcGIS Online (Content > click Form > Open in Survey123 > Open in browser), I get:

"This survey cannot be accessed in the web app. Please contact the survey owner to republish the survey with 'Create web form' enabled. Error: Missing .webform file."

Steps I’ve already tried:

  • Verified that “Create web form” was enabled during publishing from Survey123 Connect.
  • Confirmed that the .webform file is present locally in the Connect survey folder.
  • Tried re-publishing the form with various minor edits (including saving under a new name with no spaces).
  • Ran the analyzer with no breaking errors.
  • Ensured all required external CSVs (like nest_territory_lookup.csv) are present in the media folder.
  • Tried duplicating and republishing the form.
  • Confirmed sharing settings (public).

Any ideas what else could be causing the .webform to fail or how to re-enable access? Is it even possible to re-link the webform with the Survey 123 Connect form and the data records we have already entered?
