Public Survey has "It's an invalid answer" on the form despite being correct

This text appears before any answer is even given. There is a constraint on the questions and even after satisfactory answers the text remains. It also has no default set. These are text fields, required, and not part of a group.

Where did this text come from and why is it present even though my answer is correct? How do I get rid of it? It's leading people to believe they are doing something incorrect.

There is also spacing issues with my html elements in a note field:
