Not able to get a html tag for a website to work

I am trying to get a link to a website link in my survey and I am using this reference in the ESRI Notes site and I can not get this to work. Any suggestions? I completed a YouTube tutorial where I was able to create a hyperlink for a telephone #, but when I plug this tag in its place, nothing. Should I be using a specific "type" to do this? In the YouTube demo, I added the HTML tag for the telephone as a note "type" and it worked just fine.

Add in some text between the tags and see what happens,

 for instance.
