I have multiple questions in a single row, survey123, which works fine in  the form, but when I publish it to Arcgis Enterprise 11.1 the questions display one question per row.

Here is the excel screenshot, and also in the excel file, settings tab, the column form-style, it's set to field-list

How can I fix this problem?

You need to use a grid format, there are some sample templates in Connect, and here is blog post on grids:
