How to download Survey123 Connect with a Portal for ArcGIS installation

I want to start using Survey123 in conjunction with Collector, but I cannot find a way to download the Survey123 Connect application without creating a separate ArcGIS Online account.

Is there a way to download and use Survey123 Connect, or will I have to create an AGOL account?

Can you try the account username you use to access my.esri.com?

Has this issue been addressed? Through my job I have a Portal for ArcGIS publisher account, but am unable to log in with that account and download Survey123 Connect.
