How can I tick Survey123 report check boxes

Hey everyone,

In Survey123, I am currently formatting a report and am unable to tick the checkbox as shown below.

Using a direct copy and paste of the input, gives two unchecked boxes.

Please see attached spreadsheet for reference.

Any help is appreciated.

Can you please upload a copy of the template you are using?

I am having the exact same problem. Load the template then just get empty checkboxes.

Hello Henry Clark,

In regards to XLSForm and Template that you provided, I was able to publish a survey using Survey123 Connect 3.5.166 fill out a survey using the 3.5.176 of the Field App, and generate a report using the Word doc that was provided as the Feature Report template.

I have included my report result as an attachment.  Please let us know if you are still having issues on your end.

Benjamin Zandarski, please provide your XLSForm and Word doc so that we can test and determine what might be causing the issue.

Hi Brandon and Ben,

I have been able to fix this issue since posting.The problem was the Survey123 form outdated the introduction of the report function. I was able to correctly publish the report by copying the old XLSForm into a more recent XLSForm and re-publishing.

The issue we were having was using a Mac to create the checks works just

fine on Windows after testing.
