Group headings and photo icons not appearing in browser survey

I've created a survey in Survey123 Connect and the group headings and the icons for uploading an image do not appear when I access the survey in the browser but they do appear in the Survey123 app. In browser it just appears as blank space, but can be clicked, so the functionality is there, just invisible. Photos attached.

Hi Elisabeth,

I am not seeing this behavior when I create a new survey with an image question and view it on my iOS device.  Do you know what version of iOS you are using?  Can you share your form?  Feel free to e-mail me directly at jtedrick AT esri.com
