Error Message in Survey123 Web Page Background

I have seen survey online with a customize theme and image. I thought, I should give it a go. I’m using Survey123 web designer to customize our survey. I’m trying to add an image in the Web page background. However, when I tried to upload an image, I got  an “upload error” message, see below screenshot.

The image is 312 KB jpg file. Has anyone seen this error message? How did you fix it?

I also tried to use a URL to upload the image, but I got “Invalid image URL” error message.
