Error in creating survey123 with my paid portal

I have the admin right and still I am experience errors. please see error below

1. The survey does not exist or is not accessible.(This occur when i am opening my existence survey form within survey123)
2. theme is not an instance of Form Theme (This occur when you are creating new survey within survey123)

Referring to item 2 - Did you recently update your Survey123 Connect?  Are you trying to open the existing form in Connect or via the web app?

Referring to item 1 - did you recently update or modify the existing form? And same questions as above.

More information is needed before even beginning to develop a response to help solve the problem.
