Very new to survery123 but we created a range of forms for our field staff to enter and then once they return to office to upload from the sent folder to the central database. This worked fine for a month or so but now it won't send and gives the following error message on the tablets. Apologies if my descriptions are not correct, still very much getting used to the system. Many thanks in advance.
This looks like a server problem, whether AGOL or Portal depending on your setup. Review the member settings of the people trying to upload the survey and make sure they have valid current permissions to do so. Have their logins, passwords, etc changed? New people not added? And so on.

I've had this issue before, but I imagine you've checked it, I used a temporary account for the submissions of Surveys from multiple people, and that license was accidentally removed and so were the permissions. Could it be possible that the license was changed without your knowledge or possible a license expired?
If that doesn't help, I may have a few more tricks!
What may have happened is that if you published a large update (e.g., changing a text to integer, which would result in the entire data-set being reformatted), that also resets permissions for the layer. Whenever you do a large publish job, you also need to go back into the Surveys Website and update the survey permissions to what they previously were.
