Connecting polygon .csv data to survey123 form is not working. No polygon geometry field found in the feature layer

I am trying to create a survey for tracking Measles cases with polygon geometry representing the municipalities. I would like my users to choose the municipality from a drop down where the map must be able to show the corresponding municipality polygon. I have connected a .csv file which has municipality polygon vertices (WKT). I am having issues updating the form in survey123 connect since I am constantly getting an error shown below. I have also included screenshot of my xls form for the reference. Could anyone please take a look at this issue? Additionally, it is weird but the feature layer created out of this survey does not have polygon geometry field to store the polygon (municipality) selected by the user even though when I initially created this survey from sruvey123 website , I selected area option for map.
