Your question about specifying a default basemap for the survey made me research of how to do this, and after I selected a different basemap than the one currently showing in Connect, I was able to see a map in the survey page.
I didn't realize I had to do that because I was already seeing a map in Connect.
My Connect app is version 3.9.120, to answer your question. And the issue was observed in both Chrome and IE.
