Change to Survey123 Feature Report Template?

My Feature Report Template used to create a report that looked like this:

However, over the past few days, I have tried using the same template and it now does not honor the table/format of the Feature Report. I've tried uploading the template again, but I am still getting the following:

This is true whether I create the report using Survey123 or Survey123Beta. Has something changed in the Feature Report Template capabilities that would explain this change? Is there a best practice for formatting a feature report template that does not use tables?

Hi Ben,

Could you open an Esri Support Ticket?  Diagnosing how this is different will require some examination of how the Word Template file is structured.
