Want to include my custom basemap in Survey123.
My process:
-Created a .tpk using ArcPro
-added this into my media folder locally
-published the survey using Survey123 Connect
-opened the survey using the web app.
Upon firing up the Survey123 web app I can't see my custom basemap. It was set up using the settings in Survey123 Connect

Is there another option to handle this? A way to host the tpk on AGOL and just get the Survey123 web form point to my custom basemap?

The steps for associating a TPK item in ArcGIS Online with your survey are documented at Prepare basemaps for offline use—Survey123 for ArcGIS | ArcGIS . Aside from that, it is common for the TPK to be visible but the map not showing an area/zoom that has a tile - this may also cause a white map.
But the web form continues to point at the ESRI basemap, not mine
TPK sent over. Any ideas?
I have the same issue too,
what I did is:
put the tpk in media folder, and it publish, it can be shown in 'survey123 connect for ARCGIS' and on my phone app correcty, but only the webform kept showing the default ESIR map
the second I tried tiling. I uploaded tile service and changed the .info. Also I got it on my phone and the 'survey123 connect for arcgis' but the webform still stubbornly show only the defaul map

The webform will not use a TPK file.
I'm also having the same issue. Here were my steps:
1) Created then shared a tile package (.tpk) to AGOL
2) Made it public
3) Published the tiles to a hosted tile layer (also made public)
4) Edited MyForm.info with this URL format:
"http://tiles.arcgis.com/tiles/e7dVfn25KpfE6dDd/arcgis/rest/services/MyCustomMap/MapServer/tile/${z}/..." (which points to my hosted tile service)
4) Republished the survey
- It works perfectly in App & Connect; Does not work in web form.
Web form "stubbornly shows only the default map"
What am I missing here?
