Can't seem to upload/attach images via Survey 123 web form...

My new survey works fine in the app and in Connect, however, when trying to utilize a web form, I cannot submit a survey with an image attached. I receive and error message:

"Failed to submit. -Object is Missing."

Is there anyway to get past this hurdle? Here is a link to the web form:

Hi,

While this bug was initially reported when using Survey123, it was found that it affected proxied feature services in general.  This has been transferred over to the ArcGIS Online team, which is working on a fix for it.

Has there been any update to this issue?

1.  Created a survey with attachments in WYSIWYG.

2. Exported the feature service created into a file geodatabase.

3. Added the data into SQL Enterprise geodatabase.

4.Published Service into the Portal for ArcGIS(10.6.1) using the SQL table.

5. Created a survey based on the Service.

6. Can submit data into the survey via Survey123 Desktop app or Phone App.

7. Cannot submit attachment data from the web survey version of the survey123.

Hi Adam,

There apparently were 2 different issues that resulted in the same behavior.  If you are still seeing this, could you please file a ticket with Esri Support - in testing, I ma finding that some ArcGIS Servers do not support attaching an image through a feature service ApplyEdits call - I am able to replicate the behavior outside of Survey123 and without a proxied service in those situations.

James, I will be opening a ticket.  We had thought that our issue would be resolved by upgrading our Survey123Connect.  The issue to identify is that surveys will fail to submit on the web URL(pic 1).  Surveys will succeed in the other applications(Pic 2).  The database table where the feature service resides will get the "submitted" record even when the failure to submit happens(Pic 3).

It's a bit absurd that S123 for Portal cannot accept attachments for surveys.

We are also being affected by this issue:
