My new survey works fine in the app and in Connect, however, when trying to utilize a web form, I cannot submit a survey with an image attached. I receive and error message:
"Failed to submit. -Object is Missing."
Is there anyway to get past this hurdle? Here is a link to the web form:

Please give it a try again. Basically you cannot submit a survey with an attachment from the web form.
I am seeing the issue with submitting attachments. I see that you are using a proxied ArcGIS Server service - it's likely you are being affected by the same issue as in https://community.esri.com/message/729817-re-attachments-not-working-with-secure-federated-service-i...
We're seeing the same issue with our Portal set up. We have just stood up this configuration, and out first Survey seems to be working fine with data submission and when using the application (mobile/ desktop) to submit data and attachments.
However we get the "Object Missing" Error whenever we use the web form to submit an attachment.
Would you mind having a look at our survey and seeing if we're having the same proxy issues as you describe?
Would you be able to expound on what we need to do to fix this?
So is there currently no solution or work around for this issue? I have a service on our GIS server that I added to ArcGIS Online. I then pull that service into a Survey123 and receive the 'object is missing' error. It works fine within the app, but not on the web form which is where I'm looking to use it.
If you are experiencing the same issue as the others in this thread, submission with attachments should be failing for both the app and a web form. It sounds like this might be different.
If it helps any... I just found similar results with my survey created with references to an existing feature service. The web form fails to submit with error "Failed to submit -object missing" (on inspection it actually creates the record but missing the attachment) *BAD
When submitting through S123 field app, the runs successfully and record has the attachment.
Created and tested with latest V 3.3
This is a known issue - BUG-000118801 - however I assume four months in and still no fix means a fundamental flaw in the architecture of Survey 123 web forms.
