Building a signout sheet in Survey123.... any examples?

I'm looking to use Survey123 to create a sign out sheet for field equipment.  I don't need geo-reference at all, but want to keep track of who signs out GIS equipment and when it is due back.  Ideally, I would like a way to see what equipment is available on any day in a calendar format.  Does anyone know of a use case such as this?  Is there a better app to use?

Apologies for the delay in response.  Survey123 can be used to create a check-out form.  You could visualize what devices are out using the data tab, but to feed a calendar centric view you would want to migrate the data to a calendar.  One possibility might be to create a webhook that creates a new event based on the submission; an example of that (to create appointments) is featured in Massachusetts Keeps Drinking Water Safe - Esriwww.esri.com › about › newsroom › arcnews › massac...
