502 Bad Gateway publishing from Connect

Hello,
I have been been trying to publish a survey from Connect for a while and keep getting this warning. I have not been able to find a solution. I think it has to do with nested repeats because when I removed the repeats it published. I have attached the xlsx form if anyone can see something I am missing.

We have this same issue when publishing form with nested repeat to portal (v10.9.1)
